If you would like to schedule an appointment we currently utilize Phreesia online check-in system which allows you to fill out initial paperwork in the comfort of your home, or anywhere you have an internet connection prior to you visit. 2-3 days prior to your appointment you will be sent an email link (or a text link) to our pre registration patient paperwork which is now all electronic, including policy forms requiring a signature. These forms will require your medical, surgical and medication history, reason for visit, emergency contact and insurance information, the ability to take a picture of your photo ID and insurance cards, sign our policies and pay any copay prior to your visit from your phone or computer. Please gather these documents and plan on 20-30 minutes to fill out this initial paperwork. Once it is in our system any followup visits will only require a quick review of info and the ability to pay a copay or balance before seeing the doctor. We have guest wi-fi available and sanitized tablets/pads if you need to fill out paperwork in our waiting room, please plan on arriving 30 minutes early. If you currently have more that 3 daily medications please bring in a list and we will manually enter it at your appointment.
