To schedule an appointment, please call us at 706-776-3132. We will take the following information at that initial call, please be prepared with the following;
- Full name and DOB of patient (as shown on insurance to be filed)
- Email and mobile phone numbers (required for our online checkin system)
- Insurance information
We are currently utilizing Phreesia online check-in system which allows you to fill out initial paperwork in the comfort of your home, or anywhere you have an internet connection prior to you visit. 2-3 days prior to your appointment you will be sent an email link (or a text link) to our patient paperwork which is now all electronic, including policy forms requiring a signature. These forms will require your medical, surgical and medication history, reason for visit, emergency contact and insurance information, the ability to take a picture of your photo ID and insurance cards, sign our policies and pay any copay prior to your visit from your phone or computer. Please gather these documents and plan on 20-30 minutes to fill out this initial paperwork. Once it is in our system any followup visits will only require a quick review of info and the ability to pay a copay or balance before seeing the doctor. We have a secured guest wi-fi available for smartphone use if you need to fill out paperwork in our waiting room or we have a tablet/pad which can be utilized in office as well. The tablet in office is similar to texting on a phone, if you have little to no experience with texting or have issues reading smaller text, you must bring someone (friend, child, etc.) with you to fill out your registration, we do not have staff that can go through the entire registration with you. New Patients using tablet plan on arriving 30 minutes early, return patients 10 minutes. Please bring in a separate list of medications and we will manually enter your list at your appointment.
